Resumen

La diabetes mellitus tipo 2 (DMT2) contribuye al deterioro cognitivo mediante diversos mecanismos interrelacionados. El exceso de glucosa induce la formación de especies reactivas de oxígeno (ROS), lo que activa vías inflamatorias que favorecen la pérdida sináptica y el daño neuronal. La hiperglucemia también afecta a las células endoteliales y engrosa la membrana basal de los capilares cerebrales, esto genera disfunción endotelial, lo que incrementa la permeabilidad de la barrera hematoencefálica y conduce a isquemia crónica de la sustancia blanca, interrumpiendo redes neuronales esenciales para la memoria.

Además, la DMT2 se asocia con resistencia a la insulina en el cerebro, lo que altera rutas metabólicas, necesarias para la plasticidad sináptica y la consolidación de la memoria. A esto se suma el daño mitocondrial provocado por el estrés oxidativo, que reduce la producción de ATP y agrava la pérdida neuronal. Estos procesos actúan de forma sinérgica comprometiendo aún más la función cerebral, culminando en el deterioro cognitivo progresivo característico de muchos pacientes con DMT2.
